* Merge the two debug() calls in attemptQuery() into one single
call which checks DBO_DEBUG. This avoids pointless SPI logger
overhead.
* Avoid calling generalizeSQL() unless actually necessary. This
works via a lazy-loading object.
* Make lastQuery() a bit more consistent in terms of when it
gets updated.
* Add DBQueryDisconnectedError class for dropped connections.
* Clean up visbility and names of last query/error fields.

Some of the callers of setExpectations() actually need to reset the old
expectations to avoid erroneous warnings.

These comments do not add anything. I argue they are worse than having
no comments, because I have to read them first to understand they
actually don't explain anything. Removing them makes room for actual
improvements in the future (if needed).

Beyond giving a more complete picture, this can be used to adjust
the PostSend criteria to log master queries under the restInPeace()
entry point.

In logstash these messages can't be grouped in any way, because they are
all slightly different. By using a parameterized log message these can
be grouped to give a better overview about the size of a problem.
