Callers should not catch an unchecked exception, so it doesn't belong
in a function signature. Unchecked exceptions indicate a coding error,
which by definition the code will not be able to handle correctly.
If any of these exceptions were supposed to be in response to an edge
case, user input, or initial conditions, then they should be changed
to a runtime error. If the exception class cannot be changed, then
the annotation should include a comment explaining its purpose and
prognosis.

With Iee04afc27283547dd68d6db93f44ac2e0ebf1258, passing both the $class
and $factory parameter is deprecated in favor of just passing an
ObjectFactory spec as the third parameter.

This will allow constructing API modules that need services.
This overhauls some of the internals of the ApiModuleManager,
but the public interface remains unchanged.
The $class parameter of addModule, (now called $spec)
also allows passing an array with the spec of the module.
Note that this spec requires the attribute 'class' to be present,
even when 'factory' is specified. This is the same as before,
where $class was always required.
In a perfect DI world ObjectFactory would be injected into
ApiMain::__construct and ApiMain would pass that to its instance
of ApiModuleManager, but that is currently not possible, so for now
it is injected in ApiModuleManager by having ApiMain::__construct
call the service locator.

In cases where an extension has multiple API modules that
have the same exact factory requirements, it would let them
use the same factory function and check with the module manager
what the class name to construct should be. This is already possible
using getNamesWithClasses, but this makes it much more
straightforward.

This enables factory functions to be registered for API modules,
in addition to the module class itself. This allows modules to
use proper dependency injection via the modules constructor.
Example:

Which type is used depends on the ApiModuleManager responsible for
the API module. There are two managers, one in ApiMain and one in
ApiQuery. Both contain a list of API modules they instantiate.
Both use $this as the first parameter in the constructors of the
individual modules. There is no other regular way to instantiate the
modules, so we know the type must either be ApiMain or ApiQuery.
The lists don't intersect.
I would have prefered the naming scheme $mainModule for ApiMain
modules and $queryModule for ApiQuery modules but since this
doesn't add much I left the shorter variable names untouched.

Swapped some "$var type" to "type $var" or added missing types
before the $var. Changed some other types to match the more common
spelling. Makes beginning of some text in captial.

Fixed a little documentation issue, removed a line of unreachable code
and fixed up two formatting issues in the process.

Doxygen expects parameter types to come before the
parameter name in @param tags. Used a quick regex
to switch everything around where possible. This
only fixes cases where a primitve variable (or a
primitive followed by other types) is the variable
type. Other cases will need to be fixed manually.

It will allow help subsystem optimization (merging paraminfo and help),
path towards per-module or per-system versioning, removal of the
manually maintained generator lists.

* ApiModuleManager now handles all submodules (actions,props,lists) and instantiation
* ApiModuleManager maintains a cache of all instantiated modules
* Query stores prop/list/meta as submodules
* action=help suports generalized submodules (modules=query+value), querymodules obsolete
