ContentHandler::exportTransform() allows content handlers to apply
transformations upon export, such as conversion of legacy formats or
filtering of internal data.
Note that the transformation is applied to serialized content, since
the exporter will generally not unserialize the content blob to an
intermediate form before writing it to the dump. Implementations
may choose to unserialy, then transform and re-serialize.

Variants included 'in <version>', 'as of <version>' and just the
version number.
Some @deprecated annotations do not have the version number at all,
I want to hunt them down separately.

Doxygen expects parameter types to come before the
parameter name in @param tags. Used a quick regex
to switch everything around where possible. This
only fixes cases where a primitve variable (or a
primitive followed by other types) is the variable
type. Other cases will need to be fixed manually.

This allows XmlDumpWriter to output rows that are missing some
information without triggering php warnings.
This fixes warnings caused by the OAI extension when using
XmlDumpWriter::writeRevision without setting the row's
rev_parent_id field.
Generally, it seems convenient to allow optional fields to be unset
instead of skipping them if they are empty, but requiering them to be
set.

When inserting XML elements inline <such as this one>, doxygen chokes
about it not being known. Simply enclosing the tag in double quotes
prevents doxygen from emitting a warning.
Also enclosed a few invalid functions calls such as \. and double quoted
the HTML entities such as &foobar;

The content model is stored as a varbinary(32), the format
as varbinary(64).
If the standard model resp. format is used, null is written
to the database instead of the actual id, saving space.
